suggest including under SDOH. USCDI v3 includes Smoking Status under Health Status/Assessments
Suggestion
Since CACDI is more broad in it's definition compared to USCDI - I would suggest including the Health Concern TYPE similar to USCDI Problem Type. I understand the SNOMED CT code would ultimately distinguish this, but this would also make it clear to clinicians on the Chart if it is being documented as a symptom vs actual diagnosis.
Suggestion
For USCDI there is both a date of onset (could have been occurring for awhile before patient was seen) and Date of diagnosis (would technically be when entered into the system/actually diagnosed). Instead of combining an estimated or actual date, I suggest separate fields to clarify.

Suggestion to also include the Patient Health Concern Resolution Date Time. This aligns with the USCDI as well.
